The Weekly Toll: Death in the American Workplace

August 7, 2014 - Hanover, MD - Rajinder Kumar was a gas station attendant for an Exxon Gas station in Hanover when he was shot to death during a robbery.

August 14, 2014 - Wappingers Falls, NY - 53 year old Edward R. Williams was attempting to open a gate at his work place. He placed the truck in park but failed to put the brake on. The truck rolled and struck Williams, who was alone at the time. He was found the next day by co-workers of Inter-City Tire and Auto Center. 

August 15, 2014 - Bronx, NY - 25 year old Brian Jacome was performing routine maintenance on an elevator at New York Avenue Apartments. As he did so, his clothing became caught in the doors as they were closing which then descended and he also fell. He was an employee for US Elevator, Inc.

August 18, 2014 - Watertown, NY - 28 year old Christopher "Moose" Getman of Felts Mills, died on the job after he was crushed under a piece of heavy equipment as he tried to move it. He was an employee of P & M Construction of Watertown at New York Air Brake, also in Watertown.

August 18, 2014 - Sarasota, FL - 45 year old John McMahon was a maintenance worker for an unnamed company, mowing along grass on a median when a motorist struck him due to poor visibility.

August 18, 2014 - San Antonio, TX - 61 year old Ernesto Valdez was killed on the job when a bag of fracking sand, weighing 35,000 pounds, fell onto him causing major blunt force trauma and asphyxiation. He was employed by Chalk Mountain Services of TX, LLC.

August 19, 2014 - Forest Grove, OR - 29 year old Nicole Laube was an employee for Timber Creek Apartments when someone murdered her. She was found stabbed to death in a parking lot and police believe she was hanging lease-renewal notices on apartments when she was murdered.

August 19, 2014 - Tuscaloosa, AL - 63 year old David Carson of Tuscaloosa, and a line specialist for Alabama Power, and 51 year old Matthew Wallace of Hiram, GA, pilot of a helicopter for Rotorworks of Hiram, both died when the helicopter crashed. Wallace was piloting Carson to check power lines when the craft went down and was reported missing.

August 20, 2014 - Kent, WA - In a senseless act of violence, 52 year old David Christianson and 20 year old Carlos Gonzales were gunned down as they were trying to calm down an argument at the gas pumps of West Valley Shell in Kent. A customer entered the store to talk to Gonzales, who then went outside. Christianson heard a commotion and also went outside to tell everyone to calm down when a suspect went to his car, pulled a gun and shot both the employees. The gunman was charged with murder.

August 20, 2014 - Edmonds, WA - 38 year old Jason Vanlandingham of South Prairie died on the job from electrocution. He was working in a deep trench digging under an electrical duct bank and the pneumatic jackhammer he was using penetrated a buried conduit causing his electrocution. He was an employee for HOS Brothers Construction Inc.

August 21, 2014 - Kalispell, MT - 57 year old Larry Wurtz was injured on August 17 when he fell from a wheeled ladder/staircase while working at a local Walmart Super Center. He suffered traumatic head injuries and died at the hospital.

August 21, 2014 - St. Michaels, MD - 40 year old John Kenneth Shahan of Denton was controlling traffic in a work zone in St. Michaels when he was struck by a motorist.  He was a subcontractor working for State Highway Administration.

August 21, 2014 - Fort Wayne, IN - Ignacio Casillas died on the job after the tractor he was driving dropped into a manure pit, pinning him. He was an employee for Irish Acres Dairy Farm.

August 21, 2014 - Palemetto, FL - 53 year old John Winchester was killed on the job when a roller machine he was operating rolled down a berm he was working to build, cushing him. He was employed by Woodruff & Sons for Southwest Florida Water Management District at Sawgrass Lake Park. 

August 21, 2014 - Eastsound, WA - 18 year old Taylor Shane Ketchum died of complications of a viral condition while working as a camp counselor for Greater Seattle's YMCA Camp Orkila on Orcas Island. He was found unresponsive on a bunk in the staff lounge.

August 21, 2014 - Garfield Heights, OH - 45 year old Elbert Woods was killed on the job due to crushing injuries received when his clothing became caught in a commercial machine and he was pulled into it. He was working for Cleveland Heights Track Material, a railway track and components manufacturer.

August 22, 2014 - Memphis, TN - 52 year old Luis Berrocal of Costa Rico died from injuries he received in a fall. He was in a bucket lift machine trying to step onto a bridge over the Mississippi River when he fell after losing his balance. The name of the construction company he was working for is unknown.

August 25, 2014 - Oxford, IN - 45 year old Stephen Etter was killed on the job when he was struck by an industrial sized tire rim he was attempting to repair. He was trying to repair a split tire rim on a road grader when it blew off, striking him in the chest. He was an employee for a Northwestern Indiana county highway department.

August 25, 2014 - Bath, NY - 44 year old Richard J. Rowland suffered fatal electrocution leading to cardiac arrest after dumping his load in Urbana.  After unloading the dump truck, the uplifted box became entangled in overhead power lines. He exited the truck and while attempting to free the vehicle from the wires, he was electrocuted. He was an employee for the Town of Urbana Highway Department.

August 25, 2014 - Jackson, MI - 60 year old Kim Duong suffered blunt force trauma when he was struck in the chest by a falling steel plate. He was an employee for John Crowley, Inc., a Jackson steel company.

August 25, 2014 - Granite City, IL - 46 year old Rodolfo Romo of Fairmont City and 24 year old Tyler Muenstermann of Edwardsville were killed when a mortar shell exploded while working at Totall Metal Recycling. The business handles recycling of ammunition and military items but they are all supposed to be defused first.

August 25, 2014 - Rumford, ME - 35 year old Brant Munster of Newport died on the job when he fell while working on a crane. He was an employee for Somatex Crane of Pittsfield working on a crane at NewPage mill in Rumford when he hit a roof support and fell. 

August 25, 2014 - Lake Havasu City, AZ - 39 year old Charles Vacca was accidentally shot in the head at a gun range where he was an instructor. The person he was helping lost control of the weapon which kicked up and he was shot. He was an instructor for Bullets and Burgers Outdoor Range.

August 25, 2014 - Scottsdale, AZ - 41 year old Jarrod Camp and 36 year old Jeremy Johnson, both from Phoenix, died from toxic exposure while working in a confined space. The men, along with a coworker who was injured, were working inside a sewer at a Scottsdale shopping center when they were overcome and trapped. The men, who for an unknown reason did not have any safety equipment with them, were employed by Camp Industries. 

August 26, 2014 - Omaha, NE - 38 year old Bryce Dion from Santa Monica, CA was killed by "friendly" gunfire during the taping of an episode for the TV program, 'Cops'. He was working as a sound operator when police responded to a robbery at a local fast food restaurant. As police opened fire, Dion was shot. He was wearing a bullet proof vest but the fatal shot passed through an opening in the vest. He was employed by Langley Productions.

August 27, 2014 - Aberdeen, MD - 45 year old Cesar D. Caetano of North Bergen was struck and killed by a motorist as he worked on the Garden State Parkway construction. The name of the company he worked for is unknown.

August 27, 2014 - Jetersville, VA - 24 year old William Brice Seay of Glen Allen was fatally electrocuted while working on a home construction site. He had been working with a crane to put metal straps around trusses so the crane could hoist them and apparently the crane touched electric power lines in the process. He was working for his father's construction company but the name is unknown.

August 28, 2014 - San Francisco, CA - 52 year old Alvin Andrews was killed on the job when he was struck and run over by a city garbage truck.  He was part of a clean up crew for San Francisco Human Services Agency.


August 28, 2014 - Blackfoot, ID - 19 year old Bo Taylor and 46 year old Trent Sorenson died on the job from toxic exposure inside a sewer manhole. The manhole was new and clean but the use of a sealant used may have caused a toxic reaction the PVC liner emitting the toxic fumes. The young Bo was working inside the space and Sorenson may have realized something was wrong and went in after Taylor. Both men worked for Taylor Construction, owned by Bo Taylor's father.

August 29, 2014 - Albion, NY - Luis D. Larios-Hernandez was a farm worker for Kirby Farms when he was killed on the job. He was in a field standing near one truck, when another truck rolled down a hill. The truck hit him, pinning him between the two trucks.

August 30, 2014 - Christiansburg, VA - 50 year old Robert Joseph Tucker died on the job when he was struck by the bucket of a forklift. Tucker was operating the forklift & bucket and a co-worker was inside the lift working to cut trees on a property. The lift began to tip over and Tucker exited the machine and tried to run away but was struck by the bucket. The co-worker was taken to hospital, his condition unknown. It is not known who they were working for.

August 30, 2014 - Birmingham, AL - An unidentified worker died in a trench collapse while working as a plumber for an unnamed company.

August 31, 2014 - Toppenish, WA - David Gonzalez Rebolledo was killed on the job when he was pinned by farm equipment. He was towing a farm trailer, which was improperly secured. The trailer came loose from the tractor causing Rebolledo to be pinned between the steering wheel and trailer. He was an employee for Shinn & Son, Inc.
