The Weekly Toll: Death in the American Workplace

Awareness is the key in accomplishing better work place safety.  We have heard many say they just didn't know how many of our workers die on the job - until it happened to their family.  Our hearts and thoughts go out to all families who have or are suffering the loss of a loved one.

April 7, 2014 - Berkley, CA - 45 year old Damon Frick died after he fell from a motorized lift that tipped over. He was a custodian cleaning windows at University of California's International House.

April 29, 2014 - San Antonio, TX - 30 year old Aaron Turner of Virginia Beach, VA died as the result of being stung by a huge swarm of bees.  He was working for Lohmann Fencing of Houston in Grey Forest when the hive was disturbed, sending the bees swarming.

May 1, 2014 - Tanana, AK - Alaska State Troopers Gabriel "Gabe" Rich (left) from Anchorage and Patrick "Scott" Johnson (right) were shot to death during an arrest attempt of an unruly person who also caused problems the day before. Two people were arrested for the trooper's murders.

May 1, 2014 - Caldwell, ID - 53 year old Roy Fraiser of Nampa was killed on the job when he was struck by a heavy steel beam. He was using a forklift to move the beam when it shifted, falling from the lift. He was working for Rule Steel in Caldwell.

May 2, 2014 - Orlando, FL - 26 year old Brandon Bristol died on the job when he was run over by the garbage truck he was riding on. It is unclear how he came to be struck by truck.  He was employed by the City of Orlando.

May 2, 2014 - Winston-Salem, NC - 37 year old Christopher "Chris" Jones was fatally electrocuted when the elevated bucket he was in came into contact with a power line. He was am employee for Time Warner Cable.

May 3, 2014 - Ocala, FL - 30 year old Chelsea Richard, a Florida Highway Patrol Trooper, was killed on the job when she was struck by a vehicle as she was assisting in a two vehicle crash. She was speaking to a tow truck driver when another crash occurred on the opposite side of the highway, sending a vehicle crashing into her and the tow truck driver (below).

May 3, 2013 - Ocala, FL - 57 year old John Duggan died on the job when he was struck by a vehicle as he stood near the site of an accident he had responded to. Duggan was owner/operator of S.M. Duggan Towing Company and was speaking with an FHP Trooper when an multi-vehicle accident occurred on the opposite side of the highway sending a vehicle crashing into them.

May 3, 2014 - Lynchburg, VA - 63 year old Donnie Lee "DL" Bryant, Sr. of Evington died after he fell three stories when a railing he was leaning on gave way. He was the owner of DL Paint Contracting working on a Jamerson-Lewis Construction work site.

May 4, 2014 - Fairfield, CA - 77 year old Eddie Andreini of Half Moon Bay died when his stunt plane crashed during an air show at Travis Air Force Base. He was performing his Stearman Superbatics performance, flying upside down and close to the ground, when something went wrong and he crashed. He was owner of Eddie Andreini Airshows.

May 4, 2014 - San Antonio, TX - 29 year old Julian Pesina was shot to death when he was ambushed as he was closing up and left his Notorious Ink tattoo shop.  He was also a Balcones Heights Police Officer.

May 5, 2014 - Plymouth, NC - 56 year old Floyd Greene of Old Town, FL was killed on the job and another worker injured while working at Domtar Pulp Plant in Plymouth.  He was standing in front of a hydraulic line that pressurized and suddenly burst. He was contracted through CBT Services, Inc. of Chattanooga, TN.

May 5, 2014 - Genoa, IL - 72 year old Leon Madey was found dead inside a grain silo after his family realized he was missing. He was helping to dislodge grain on the family farm when be was buried.

May 6, 2014 - Shawnee, KS - 21 year old Nicholas Huston of Edwardsville died from injuries he received after falling out of a golf cart at the FedEx facility in Shawnee where he was working. It is unclear why he fell from the cart.

May 6, 2014 - Corona, CA - Samuel Enciso of Santa Fe Springs, was killed when an explosion occurred as he welding on a petroleum tanker truck. A co-worker was injured. The blast occurred at National Distribution Services in Corona.

May 6, 2014 - Ardmore, OK - 31 year old Christopher DeWayne Cryer of Lone Grove died after falling approximately 15 feet from a plant in Ardmore. He suffered fatal head trauma. He was employed by Atlas Roofing.

May 7, 2014 - Orlando, FL - 29 year old Keith Bramblet died after falling or being thrown from an elevated cage attached to a crane. He was working from the cage to install an electronic billboard. A witness states the cage was swaying so much that it also came apart from the crane but the reason is unknown at this time.

May 8, 2014 - Delmar, IA - 75 year old Donald Cain was killed on the job when he was pinned in farm machinery in rural Delmar, near Maquoketa.

May 9, 2014 - Scottsbluff, NE - 67 year old Llewellyn Mattern died after falling about 11 feet down into a ditch on fire. He and his Pathfinder Irrigation District crew were burning weeds in the ditch when he fell in and was unable to get out.

May 9, 2014 - Indianapolis, IN - 49 year old Kenneth Duerson, Jr. of Indianapolis and 24 year old Coty Demoss of Noblesville, we both killed on the job when they were struck by a motorist while removing cones during road construction.  They worked for Rieth Riley Construction Co., on an Indiana Dept. of Transportation "Operation Indy Commute" project.

May 9, 2014 - Howard, SD - 66 year old James Henry Rentschler died after he fell from a skid steer he was operating and was pinned between the cab and arm of the machine on his farm.

May 9, 2014 - Arlington, WA - 66 year old Donald Monroe Long died after suffering a fatal heart attack while working from a patient's home.  He was employed by Fred and Debbie Inc. dba Visiting Angels Living Assist.

May 10, 2014 - Zeeland, MI - 49 year old Ricardo Ramos Grand Rapids was killed on the job when he was caught in a piece of machinery.  He was working at the meat packing plant Hillshire Brands, performing maintenance, when his clothing somehow became caught in a conveyor belt which caused asphyxiation.

May 11, 2014 - El Paso, TX - 54 year old Jimmy Joe Broadstreet of Chaparral and 39 year old James Green of Runaway Bay, were struck by a drunk driver who drove through road construction barricades. Green died on scene and Broadstreet at hospital. It is unclear if they were employed by the Texas Department of Transportation or a subcontractor.

May 12, 2014 - Boone County, WV - 46 year old Gary Hensley of Chapmanville and 48 year old Eric D. Legg of Twilight were killed on the job when a mine roof collapsed during a retreat mining operation at Patriot Coal's Brody Mine No. 1 in Boone County.

May 12, 2014 - Savannah, GA - 66 year old Bounpheng Srirtanakoul died after his chest was crushed by a large pipe fell on him while working on a repair project at Savannah's city waste water treatment plant. He was a senior mechanic for Savannah City.

May 12, 2014 - Brentwood, NH - 48 year old Steve Arkell, an officer for the Brentwood Police Department, was shot after responding to a home for a domestic dispute. The suspect then set the home on fire and an explosion occurred.

May 12, 2014 - Minot, ND - 48 year old Richard Rayburn of Hillsboro, OH died after he fell from a roof at a construction site in Minot. He had just unhooked his harness to move position when a gust of wind caused a piece of plywood to blow up into him. As he tried to catch it, he stepped on another piece that caused him to slide off the roof. It is unclear who he was working for.

May 12, 2014 - New Philadelphia, OH - 29 year old Michael C. Cottrell died at a hospital after he was seriously injured in an explosion. He was inside Sutton Pump and Supply Company welding on a bulk storage tank when the explosion occurred.

May 12, 2014 - Lansing, MI - 35 year old Michael Addo of Mason, and a customer, were shot to death inside the Rite Aid store Addo was working at.  It appears there was no other motive than a nearby resident was angry over a domestic dispute so he entered the store and started shooting.

May 13, 2014 - Merritt Island, FL - 43 year old Brian Boyle died on the job after he was struck by a tree limb that fell as another worker was up in the tree, trimming it. He was standing on the ground when the incident occurred. He was employed by Gidney's Tree Experts.

May 13, 2014 - Fort Wayne, IN - 26 year old Robert "Bobby" Houser, Jr. died after he was injured May 9 when the riding lawn mower he was driving flipped, pinning him in a pond. He worked for Adams Landscaping according to his obituary.

May 13, 2014 - Withee, WI - 55 year old Roger R. Petranovich was killed on the job when the dump truck he was driving went off the roadway and rolled into a ditch.  He was working for HAAS, Inc.

May 14, 2014 - Houston, TX - An unidentified driver of a tractor trailer died on the job when the rig rolled into him at a loading dock of a Kroger Grocery Store. It is unclear who he was working for and attempts to locate his name have not been successful.

May 14, 2014 - Tacoma, WA - Anthony McPeek of Puyallup suffered a fatal heart attack while in his company truck in a parking lot.  He was employed by Acco Engineered Systems, Inc. at Tacoma Art Muse.

May 14, 2014 - Puyallup, WA - Richard Phillips collapsed at work from a heart attack.  He was working for Alteon Training LLC dba Flightsafety Boeing Training.

May 14, 2014 - Spanaway, WA - Dennis Eaton died of a heart attack 10 days after he fell from his tractor trailer semi as he was loading it with personal items for an upcoming trip. He was hospitalized until May 13 due to the fall but died in his sleep the next day. He was employed with Aggressive Transportation Ltd USA.

May 15, 2014 - Louisville, KY - 56 year old Craig Smith of Cincinnati, OH died of injuries he received when he fell the day before from the top of a tanker truck trailer, striking his head. He was working at Apollo Oil.

May 15, 2014 - Venice, FL - An unidentified construction worker died after falling from a 25 foot high steel beam, striking his head. He was working at the Tervis Tumbler expansion construction site. It is unclear if he was working for them or a subcontractor.
