The Weekly Toll: Death in the American Workplace

Our thoughts and hearts go out to the family of these workers who did not go home after their workday.  Please let us know if we can assist you in any way.

December 22, 2013 - Longview, WA - 68 year old James Nels Dukart of Damascus, OR died on the job when a truck that was apparently left in reverse rolled back, striking him.  His company is not named but he was working at the Duke Construction & Excavating, Mint Farm Industrial Park construction site with his partner, son Steve Dukart.

December 23, 2013 - Olympia, WA - 37 year old Michael Webber of Hoquiam was killed on the job while driving his logging truck for Lutton Trucking, Inc.  He failed to negotiate a curve down a hill and went off the road. His truck struck a stump causing his load of logs to shift forward and into the cab of the truck.  

January 16, 2014 - Everett, WA - 52 year old Gregory Billen died as the result of natural causes while working on his recycling vehicle for Waste Management of Washington, Inc.

January 16 - Elkhart, IN - 20 year old Krystle Dikes was killed on the job when she was shot to death by a man who entered the store and opened fire on her and a customer.  She was employed by Martin's Supermarket.

January 16 - Manhattan, KS - 47 year old David Teske of Onaga died on the job when he was struck by a falling tree at a logging site near Manhattan.  His employer is unknown.

January 16 - St. Petersburg, FL - 60 year old Terry Lynn Flack of Brooksville died on the job after he fell about 30 feet from a catwalk on the Eckerd College Campus. He was an electrician employed by Universal Controls. 

January 17 - Seattle, WA - Teresita Acido was found unconscious and died of a heart related issue while working at Aramark Sports Safeco Field.

January 18 - New Bedford, MA - 35 year old Victor Gerena was killed on the job when he was crushed after becoming entangled in a shellfish shucking machine.  He was employed by New Bedford Sea Watch International. 

January 18 - Fairburn, GA - 21 year old Tim June of Roswell died on the job while working a tree trimming job for his company, Atlanta Tree Assurance.  In a series of events during cutting a tree he was struck by a limb and then became tangled in his safety line, which ended up causing him to become strangled. 

January 18 - Burlington, IA - 52 year old Jay Ehret died hours after falling from the roof of an apartment building.  He was reaching for his nail gun, when he slipped and fell.  It is unclear who he was employed with. 

January 19 - Bensenville, IL - 36 year old Hussein Saghir was shot to death during a robbery attempt as he and his brother, owner of Sam's Tobacco & Food Mart, were preparing to lock up the store and leave for the night. Saghir worked as a clerk for the store.

January 20 - Madill, OK - 23 year old Brody Davenport of Durant and 38 year old Gonzalo Martinez of Madill died of thermal injuries and smoke inhalation due to an explosion in a furnace that occurred inside Mid American Steel & Wire plant in Madill.

January 20 - Fall River, MA - 47 year old Allen L. Jarabek, Jr. of Swansea died after a commercial truck he was working on rolled over him.  He was owner of Ground Earth, Inc. and Dover Towing.

January 20 - Milan, NH - 42 year old Brian Young of Concord, VT was killed on the job when he was struck by a logging truck driven by his co-worker.  The co-worker believed Young was clear of the truck before moving forward and did not see him.  Both worked for Kelley Trucking of Milan. 

January 20 - Omaha, NE - 53 year old Keith Everett and 47 year old David Ball were killed, and ten others injured, when a blast caused the collapse of much of the International Nutrition plant in Omaha.

January 20 - Red Bud, IL - 44 year old Jeffrey Guebert died on the job after falling into a grain bin filled with corn.  He had not been seen by others for half an hour before being found in the bin. He was employed by Guebert Farms.

January 20 - Bronx, NY - An unidenfied driver of an 80,000 pound cement truck died after his truck veered off the roadway for an unknown reason, slamming into a apartment.  Repeated attempts to locate the driver's name or employer have been unsuccessful.

January 21 - Sully, IA - 26 year old Willie Mercer died of possible toxic exposure inside a tank trailer that may have contained a hazardous material.  He and two other workers passed out while inside the trailer.  All worked for Sully Truck Wash at the time of the incident.

January 21 - Dublin, CA - Sgt. Tom Smith, a Bay Area Rapid Transit officer from San Francisco, was shot and killed by friendly fire during a probation search of a suspect's home.  The other officer's weapon either accidentally discharged or he thought he saw someone else. The bullet passed through a gap in Smith's protective vest.

January 21 - Otay Mesa, CA - An unidentified man was killed when he became pinned between two tractor trailer rigs at Mex-Cal Truckline, Inc.  He was changing a tire on one of the rigs when it fell off the jack, pinning him against the other truck.

January 22 - Phoenix, AZ - Two unidentified employees for Riddell Painting Company were killed when the lift truck they were working in toppled over at the Lower Buckeye Jail facility.  The lift may have been parked on a slope.   

January 22 - Colorado Springs, CO - 41 year old Pedro Arrazola died on the job when he fell from a fire escape while inspecting a roof.  He was employed with Weathercraft Roofing.

January 23 - Raleigh, NC - 30 year old Gabriel Andrade died on the job when he fell from scaffolding at an apartment construction site. He was wearing a harness and lanyard but apparently unhooked it to lean outward to install a soffitt.  He was employed by Exterior Recovery, Inc. of Youngsville. 

January 23 - Davenport, WA - 54 year old Warren Wade Damschen died of toxic exposure while working inside a confined space septic tank.  He and other workers were cleaning out a septic tank when Damschen descended into the space to shovel excess waste to the hose. After calling for a ladder he did not appear and other workers used a hose to pull him out.  He was employed by Short Larry Harmon dba Short Septic Service & Contracting.

January 24 - Clarksville, AR - 33 year old Daniel Rice was killed in a tank explosion at his job at Quick Transports, Inc.  He was using a torch to thaw the tanker when it exploded and he was struck by a metal hatch plate blown off the back of the truck.

January 24 - White Pigeon, MI - 30 year old Eric Rodman of Elkhart, IN was killed on the job when he fell into one of the hopper railroad cars he was loading.  He was working for The Anderson's Grain and Storage facility in White Pigeon.  

January 24 - Mentone, IN - 42 year old David M. Seitner died from injuries he received when an air compressor system cover on a truck exploded in his face.  He was working for RVH Trucking at Cargill Nutrina Feed plant. 

January 24 - White City, OR - An unidentified 42 year old construction supervisor for Washington State based Global Pacific Environmental died on the job after being pinned to a wall by a dump truck. Global Pacific was working under Texas based Burton Construction.

January 24 - Salina, NY - 50 year old Rodney Dolittle died on the job when he suffered a heart attack while driving a taxi and crashed into a tree.  He worked for a company who uses the names Yellow Cab, Ace Taxi Service and City Taxi.

January 25 - Columbia, MD - 21 year old Brianna Benlolo of College Park and 25 year old Tyler Johnson of Mount Airy were both shot to death by an armed man who entered their Zumiez Skate Shop in The Mall of Columbia and opened fire on them.

January 25 - Gainsville, GA - 39 year old Jose Luiz Martinez was killed on the job when he fell from the elevated bucket he was working from. He was not wearing a safety harness and it appears part of a tree he cut bounced into the bucket, causing it to sway enough that he fell out. He was employed by MTZ Tree Service. 

January 26 - Toledo, OH - 42 year old Private Stephen A. Machcinski and 31 year old Private James A. Dickman died on the job of thermal burns and toxic exposure while fighting an apartment fire in Toledo.  Both were members of Toledo Fire & Rescue.

January 27 - Aurora, IL - Vincent Petrella, an Illinois Tollway employee, was killed on the job when he was struck by a semi truck as he was assisting a state trooper with another disabled semi truck on the Reagan Memorial Tollway.  The officer was injured.

January 27 - Silt, CO - A helicopter crash claimed the lives of three men in Colorado.  The men were inspecting power lines for Holy Cross Energy when the blade of the helicopter touch a power line, causing it to crash into a ravine.  Following are those who lost their lives in this incident:

40 year old Chris Gaskill of Aurora was a heat inspector for Hot Shot Infrared Inspections.
51 year old Larry Shaffer of Rifle was a crew foreman for Holy Cross Energy. 

January 28 - Trumann, AR - 53 year old Rick Winkles died on the job from an apparent heart attack while working at the scene of a fire.  He was Fire Chief of Trumann Fire Department.

January 28 - Waco, TX - 55 year old Jose Dario Suarez of Manor died on the job and his co-worker injured when the elevated boom, or manlift they were working in detached from a bridge they were working on and fell into the Brazos River. They working for Derr & Isbell Construction on a pedestrian bridge at Baylor University campus. 

January 28 - Cottage Grove, OR - 49 year old Santiago Giron of Phelan, CA was killed on the job when the semi truck he was driving rolled as he approached a stop sign.  It is unclear who he was employed by.

January 28, Westford, NY - 40 year old Daniel Banks died on the job when he became entangled in a mechanized corn chopper on a farm. The machine's power take off was frozen and then became engaged. Banks clothes were caught in the moving part. 

January 28, Pennington, PA - 33 year old Christopher Stiff of Newfoundland was killed on the job when he was crushed by a load of steel beams on his truck.  He apparently tried to stop too fast causing the load of beams to shift forward, moving into the cab of his truck.  His employer is unknown.

January 29 - Fries, VA - 55 year old Dennis Carlie Johnson died on the job after he was pulled into a conveyor belt.  He was attempting to clear lumber at C & B Lumber when his clothing was caught by the belt and he was pulled in. 

January 30 - Spanish Fork, UT - 44 year old Cory Blake Wride was killed on the job when he was ambushed and shot to death by a gunman as he was checking on what he thought was an abandoned vehicle.  He was a Sergeant for Utah County Sheriff Department.

January 30 - St. Mary's, PA - 52 year old Arwed Ralf Uecker of Boonton, NJ was killed on the job in an explosion of manufacturing equipment (an oven) at the Mersen USA plant in St. Mary's. Uecker was global director of research and development for Mersen based in Boonton.

January 31 - Woodlawn, MD - 54 year old John Kahl of Port Deposit was killed on the job when he was struck by a motorist while working along a section of roadway construction.  He worked for a subcontractor for State Highway Administration. 

January 31 - Indianapolis, IN - 30 year old Daniel Jaffke died after being shot by someone who then stole his vehicle.  Jaffke, a delivery driver for Papa John's Pizza, had made a delivery to a resident at an apartment complex and was confronted as he went back to his vehicle.
